Generally emails are not sending out because you have setup custom email settings and the information is incorrect. Email settings can be found under Company > Customize Application > Domain settings > Email Domain panel.
If your application isn’t sending emails then you need to verify your email settings. Most common mistakes are not setting the SMTP type based on email service, not including the full username, or not specifying a From Name or From Address.
It is also generally best to have the from name and from address match the email account you are sending from. If the From Name and From Address don’t match the email account associated with the username spam filters may block the emails from being sent or received.
